  • You will need a computer or smart device with internet access.
  • You will also need a recent receipt from the store.
  • You need to be able to read and understand English or Spanish to take the survey.
  • You will need to provide some personal information, such as your name and email address.

  2. If you prefer to take the survey in Spanish, you can press the ‘Espanol’ link at the bottom. You will be redirected to the survey in your preferred language.

  4. Go ahead and enter the time, store number, and 15-digit survey code.
  5. Press the start button, and answer the questions to the best of your ability.
